Assistant Director, Care Transition
To lead a team and oversee daily operations and performance to ensure efficient, effective and professional delivery of referral and AIC Link services.
• Plan and optimise team resources to ensure efficient and effective management of referrals, enquiries, service delivery and KPI achievement.
• Operationalise policies, procedures, schemes and services, ensuring staff and stakeholders are aligned and compliant.
• Supervise, coach and develop staff, including performance management, capability building and career development.
• Provide guidance for escalated cases, complaints, feedback management and service recovery to achieve positive client outcomes.
• Assess client needs, facilitate referrals and care planning, and coordinate with stakeholders to ensure timely access to appropriate services.
• Analyse referral trends, identify service gaps and operational issues, and ensure accurate documentation and compliance with governance requirements.
• Lead and support strategic initiatives, pilots and process improvements to enhance service delivery and operational effectiveness.
• Collaborate with internal and external stakeholders, to support operationalisation, care coordination and integrated service delivery.
Job Requirements:
• Professional qualification in Nursing, Social Work, Allied Health disciplines or a similar field along with relevant healthcare experience
• Minimum 12 years of experience in related field in acute or community healthcare setting, or community care setting in Singapore, with at least 6 years in a managerial capacity
• Computer literacy with proficiency in MS Office applications and basic statistics
• Exhibit high degree of flexibility, adaptability, assertiveness, maturity and confidence
• Positive-minded, critical thinker, self-directed, able to work as part of a multi-disciplined team, and have good interpersonal and communication skills
• Ability to manage under pressure and take initiative; demonstrates leadership skills
