Job Description

 

  • Programme Manager of Community Mental Health Master Plan Programmes (ASCAT and Aftercare)
    • Analyse service gaps and develop enhanced model of care to augment existing services in the community.
    • Collaborate with the internal and external stakeholders (restructured hospitals and RHS) to achieve alignment on project outcomes, deliverables, and implementation plans.
    • Conduct ongoing review to develop enhanced model of care to support clients requiring mental health care support.

 

  • Account manager to establish and sustain partnerships with identified stakeholders to define the principles and guidelines within the eco-system for collaborative partnership and innovative care model.
    • Engage stakeholders to identify opportunities for collaboration and work with partners for capability building of the mental health ecosystem in accordance to the stepped care model competencies.
    • Work closely with HOD and the RHs in formulating plans to develop the capability of partners in community health sectors.
    • Support regional engagement with the Regional Health Systems, restructured hospitals, polyclinics, GPs and community partners to support population health objectives, strengthen health and social integration and streamline client referrals.

 

  • Establish a sustainable financing and resourcing framework.
    • Work with service providers on the timely submission of KPIs and claims for processing and reporting.
    • Develop and put in structure for continuous service and process improvements.

 

 

Job requirements

  • Bachelor’s Degree, Post Graduate Diploma, Professional Degree preferably in Social Work/Psychology/Counselling.
  • At least 2 years’ relevant experience and familiar with multi-agency work/healthcare industry, programme implementation and/or programme management.
  • Experience and knowledge in facilitation, design thinking, organization development preferred.
  • Experience in community development and multi-agency work in healthcare/social care industry would be an advantage.
  • Good team player and willing to embrace and manage change.
  • Able to multi-task and work well under pressure.
  • Able to work independently and as a team player to collaborate with various stakeholders, internally and externally, with motivation to learn.
  • Meticulous and inquisitive attitude with strong follow-up skills.
