Assistant Manager/ Senior Executive, Program Management (CCMS)

This position is responsible for programme administration of Community Case Management Service (CCMS), which may include administration of MOH funding for appointed CCMS service providers (SPs), account management to support CCMS SPs to achieve the objectives and key deliverables of the Programme as well as driving capability building efforts to improve service quality of CCMS SPs.

 

Key Responsibilities and Desired Outcomes

Programme Administration

  • Work with CCMS service providers, community partners and referral sources to achieve programme objectives and improve programme deliverables.

  • Administer MOH funding to service providers for CCMS programme by monitoring workload and make adjustment as required.

  • Track programme budget utilization.

  • Track programme KPIs for reporting to MOH and AIC.

  • Review project regularly to identify and understand gaps and issues in existing service. 

Account Management

  • Establish strong rapport and close communications with CCMS service providers to improve deliverables of CCMS programmes.

  • Support service providers on operations related matters, e.g. referral management, admission workflow, service requirements.

Capability development for CCMS service providers

 

  • Drive capability building efforts for CCMS case managers through organising of cross-organisations case discussions with AIC Clinical Consultants; and arrangement of interRAI training.

Any other duties as assigned by Reporting Officer/Director/Chief.

Requirements: 

  • Recognised Degree in any discipline

  • Experience and knowledge in health and/or social care, or programme implementation will be a plus.

  • Strong organisational skills in communication, time management, analytics, critical thinking and problem solving.

  • A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
